Case Manager, STRC

Multicultural Care is a non- government, not-for-profit, secular community-based organisation that specialises in providing person-centred, consumer directed home care services to frail-aged people and their carers from CALD backgrounds.

Reporting to the Manager, Client Services, you will play an important role in managing the delivery and development of STRC services and ensures services operate to budget.  You will be responsible for delivering high quality, person-centred packaged care, with a primary focus on Short Term Restorative Care (STRC packages).  STRC packages have a number of key features.  They provide:

  • A goal oriented, multi-disciplinary package of care designed to meet the client’s needs to help them stay at home and help defer entry into higher levels of care
  • High intensity short term period of care (up to a maximum of 56 days) to address or improve functional decline
  • A multi-disciplinary care approach via multi-disciplinary teams (MDT)
  • Interventions that support and encourage the client to make the most of their own capacity and achieve their full potential, with the aim of minimising care needs over the longer term

  • Establish STRC program
  • Manage a case load of client’s in-line with the industry benchmark
  • Maintain a strong focus on client outcomes
  • Manage compliance

Skills, Experience and Competencies

Essential Criteria

  • Diploma level qualification in Aged or Community Care, Case Management or related field, and/or experience in similar capacity
  • Demonstrated experience in working with people with complex needs
  • A genuine commitment to providing high quality, person-centred aged care, and to the values and philosophies of Multicultural Care
  • Demonstrated understanding of needs and issues of older people from CALD background and their carers
  • Case Management experience especially in short term restorative care g. Transitional Care/ Compack
  • Ability to maintain and promote and independence-focussed approach to assessment and development of goal based care plans.
  • Proven ability to develop and implement effective care plans which address client needs within individual budget constraints
  • Effective communication, negotiation, organisational, time management and customer service skills
  • Good administrative and computer skills
  • Demonstrated ability to coordinate and work as a part of a multidisciplinary team
  • Ability to support Bilingual Support staff
  • Access to own vehicle
  • Current First Aid Certificate or a willingness to obtain

 

Desirable Criteria

  • Registered Nurse or Allied Health Qualification
  • Ability to speak second language
