The Australian Government has announced the Support at Home program will commence on 1 November 2025. By now you should have received your new Service Agreement. Please sign and return this by 28 October. You can call My Aged Care on 1800 200 422 to check who is currently listed as your representative and familiarise yourself with the proposed new Registered Supporter roles and responsibilities.

What is the Support at Home Program?

Support at Home is the new name for the in-home packaged care services you may already be receiving from us. It will combine services from the current Home Care Package Program and Short-Term Restorative Care programme.  

Can I still adjust my care package?  

Yes. Multicultural Care is committed to providing you with continued flexibility to meet your assessed care needs. 

Will I still receive my current HCP services? 

Yes. You will continue to receive care and services as agreed with Multicultural Care. Any HCP funds not spent will still be accumulated and rolled into the next financial year. From November 1 HCP and STRC services will be combined under the new Support at Home program with up to 8 levels of service offerings. 

What services are available under Support at Home?
Clinical Supports: Specialised services to maintain or regain functional and/or cognitive capabilities Nursing, most allied health, nutrition, care management and restorative care management 
Independence: Support to manage activities of daily living and loss of skills or function to live independently Personal care, social support & community engagement, therapeutic services, respite, transport, assistive technology, home modifications 
Everyday Living: Support to keep your home in a liveable state and support your independence at home Domestic assistance, shopping assistance, home maintenance (including light gardening) & home repairs, and meals (preparation and/or delivery) 
Can I do anything to prepare for November?

Yes. By now you should have received your new Service Agreement. Please sign and return this by 28 October. It’s October now, what should I do? 

Start thinking about changes you might want to make to your level of care and potential funding. This will ensure your new care agreement meets your needs. It is a Government requirement for you to sign a new agreement and we will contact you in September to start this process.

Am I a Grandfathered or Hybrid client?

The Australian Government has made the decision to categorise people receiving aged care services as either Grandfathered or Hybrid clients.  If you were approved for your Home Care Package on or before 12 September 2024, you are considered Grandfathered and will move to the Support at Home program on 1 November 2025.  

If you were approved for your Home Care Package after 12 September 2024 – you are considered a Hybrid client.
