How to access NDIS

Multicultural Care is currently registered for seven supports targeted at assisting people with a disability.

The NDIS Access Request Form is the official document you need to complete to apply for NDIS support. This form helps the NDIS determine your eligibility based on specific criteria including age, residency, and the nature of your disability or need for early intervention. You can find it via ndis.gov.au and you need to submit it along with your evidence. If you need help submitting your NDIS Access Request Form you can call 1800 800 110 or contact your local NDIS office.

The National Disability Insurance Agency (NDIA) will then contact you for a meeting with a Planner, where you will discuss your goals, functional capacity and determine the supports you need.

The NDIA will approve your NDIS Plan, which is a budget for supports and services the NDIA decided will assist you to live an ordinary life. The cost limit of each support is determined by the NDIA.

There are options for you to choose regarding who manages finances within the budget, e.g. the NDIA or a Plan Manager. You can choose your preferred providers for all the supports you are funded for within your Plan.
