Who’s eligible for home-care?

If you’re over 65 and living in Australia – you are likely eligible for free or subsidised home care services. How good! Duō hǎo a! Kam hu jid! Queu beuno! Thật tốt!

What type of help can I get?

Depending on your situation and financial status, you could start receiving daily personal care, private travel to appointments, help with cooking and shopping, physiotherapy and occupational therapy and even supported community activities and outings to stay connected and meet new people.

FAQs

What is the Support at Home Program?

Support at Home is the new name for the in-home packaged care services you may already be receiving from us. It will combine services from the current Home Care Package Program and Short-Term Restorative Care programme.  

Yes. Multicultural Care is committed to providing you with continued flexibility to meet your assessed care needs. 

Yes. You will continue to receive care and services as agreed with Multicultural Care. Any HCP funds not spent will still be accumulated and rolled into the next financial year. From November 1 HCP and STRC services will be combined under the new Support at Home program with up to 8 levels of service offerings. 

Clinical Supports: Specialised services to maintain or regain functional and/or cognitive capabilities Nursing, most allied health, nutrition, care management and restorative care management 
Independence: Support to manage activities of daily living and loss of skills or function to live independently Personal care, social support & community engagement, therapeutic services, respite, transport, assistive technology, home modifications 
Everyday Living: Support to keep your home in a liveable state and support your independence at home Domestic assistance, shopping assistance, home maintenance (including light gardening) & home repairs, and meals (preparation and/or delivery) 

The Australian Government has made the decision to categorise people receiving aged care services as either Grandfathered or Hybridclients.  If you were approved for your Home Care Package on or before 12 September 2024, you are considered Grandfathered and will move to the Support at Home program on 1 November 2025.  

If you were approved for your Home Care Package after 12 September 2024 – you are considered a Hybrid client.

Wendy, 74, lives at home and values her independence, but she now needs regular support with some everyday tasks.

  • Level 2 support helps her remain safe, comfortable, and connected while continuing to live in her own home.
  • A support worker visits Wendy a few times each week to help her freshen up and try on new clothes. She also has help with household tasks including cleaning, laundry, and meal preparation, which supports a healthy diet and a safe living environment.
  • Keeping social is a no-brainer for Wendy and… apparently, it’s great for the brain! Companionship and outings support her emotional wellbeing.
  • Wendy benefits from simple reminders to take her medications and help keeping them organised.
  • Basic home and garden maintenance, along with regular check-ins, help ensure her safety and overall wellbeing.

Emad, 80,  has advanced Parkinson’s disease and mild cognitive decline. 

  • With his level 4 package, Emad receives a culturally respectful care plan that supports his independence and wellbeing at home.
  • A support worker visits daily to assist with personal care, including showering, dressing, and grooming, helping him start each day with dignity.
  • Medication support is provided to ensure correct dosages and coordination with his pharmacist. Several times a week, Jamal visits to help keep his home clean, safe, and comfortable, while daily meal preparation focuses on nutritious meals that reflect Emad’s cultural preferences.
  • Regular physiotherapy sessions support his strength, balance, and mobility to reduce the risk of falls.
  • Transport assistance allows Emad to attend medical appointments and community or social outings, helping him stay connected.
  • Home safety modifications, including grab rails, a walk-in shower, and a mobility aid, further support Emad to live safely and confidently in his own home.

Jan, 88, lives alone in her own home, which she wishes to keep, but it requires modifications for safety.

  • Because of her complex needs, Jan’s care plan focuses on avoiding hospitalisation through consistent clinical and personal support.
  • Health Status: Chronic, complex conditions (e.g., moderate-stage dementia, Type 2 diabetes requiring insulin monitoring, and reduced mobility from severe arthritis). 
  • Key Care Needs: Daily assistance with personal hygiene, medication management, and specialized mobility support (hoist transfers). 
  • A registered nurse visits 3–4 times a week for complex wound management, blood sugar monitoring, and insulin administration. 
  • Regular routine visits from a physiotherapist to improve strength and mobility, and an occupational therapist for home safety assessments (e.g., bathroom modifications, mobility aids). 
  • A Multicultural Care support workers visit daily to assist with showering, dressing, and continence care. 
  • Jan also receives weekly cleaning, meal preparation (specifically for diabetic management), and laundry services.
  • Her level 6 funding includes transportation & social support like accompaniment to medical appointments and support to attend local community activities. 
  • A dedicated care coordinator who liaises with GPs, specialists, and family.
