Home Care Package
Funded by the Commonwealth Government, Home Care Packages (HCP) provide long term support for older people aged 65 years and over, and Aboriginal and Torres Strait Islanders 50 years and over – so that they can maintain their independence at home. Packages are designed to be flexible and on demand – in line with the government’s new consumer directed care approach (CDC). Under Consumer Directed Care (CDC) the consumer/participant can choose the services they want, how and when they want them delivered – and by a preferred service provider.
A Home Care Package enables consumers/participants to personalise support services so they can live a healthy, safe and independent life in the comfort of their own home and to stay connected to their community.
Home Care Packages are available in four levels of care.

Home Care Package?
The Home Care Package can be used for products and services that support independence at home, health and well being and social connections in the community. Services may include:

Am I eligible?
To find out if you are eligible you must contact My Aged Care who will organise an assessment through the Aged Care Assessment Team (ACAT). If you are eligible for a package, you will receive specified funds/resources and, together with the assistance of a dedicated Care Advisor, you can select and purchase services to best meet your care needs or wishes.

How to access a Home Care Package
Home Care Packages are funded by the Government and are available to anyone assessed as eligible by the Aged Care Assessment Team (ACAT).
Stage 1
Call ACAT (Aged Care Assessment Team) on 1800 200 422 to lodge your interest in receiving a home care package.
Stage 2
Our team receives your homecare enquiry (either through a referral from My Aged Care or privately for self-funded individuals). We discuss your care requirements, goals and preferences with you. We provide information on the package services available to you depending on your level of care needs and costs.
Stage 3
We arrange for our specialist Care Advisor to visit you and carry out a comprehensive care assessment of your care needs, including a risk assessment to ensure your safety. Following this visit, a fully costed care package will be offered.
Stage 4
We finalise your care package and develop your plan with you and your family, making sure you are happy with every aspect.
Stage 5
We identify a suitable staff member to support you and arrange for you to meet them to ensure that all of your needs are discussed based on your wishes and care plan. If any additional allied services such as physio, podiatry or nursing service is required to support your care requirements, this will also be coordinated.
Stage 6
Your service commences and a monthly statement is sent to show services received and how your budget is allocated.
