Support at Home Program

Live independently at home, on your own terms with extra support as your needs change.

Under Support at Home, Clinical care like nurse visits, physiotherapy, podiatry and occupational therapy are client contribution free.

You won’t have to co-contribute (out of pocket) for all clinical services from nursing to allied health. The amount of subsidised services you can access will depend on your individual situation like whether you are a full or part pensioner or self-funded retiree. People applying for the first time, will need to be assessed by My Aged Care – and we can help you with that.

From 1 October 2026, services such as personal care like showering, grooming or help using the toilet will also be contribution-free.

What you need to know

Long term support

Long term is for people who need more regular, coordinated, or higher-level help at home. Under this program there are 8 primary funding levels available. Your level is determined by your specific care needs and your living situation.

Services are broken down into three categories Clinical Care, Independence Support and Everyday Living.

  • Clinical care like nurse visits, physiotherapy, podiatry, and occupational therapy
  • Independence support like grooming, showering, medication help, transport, respite, group social activities
  • Everyday living like cleaning, gardening, shopping and meal preparation

Short term support

There are 3 additional short-term support pathways available (assessed and funded separately to your Long-Term Support at Home funding level)

Assistive Technology and Home Modifications
  • Modifications made to my home to improve access and independence
  • Technology to help me communicate and continue with my normal activities
Restorative Care Pathway
  • Up to 16 weeks of additional support
  • Designed for people recovering from illness, injury or declining health but wish to stay at home
  • Aims to achieve specific, individualised goal to rebuild strength and independence
End of life pathway
  • Access to $25,000 for a maximum of 16 weeks of extra support
  • The services you can receive are the same as those available under Long Term Support at Home
  • Works alongside any government-funded palliative care they may already receive

What you may pay

  • Clinical care: you pay $0
  • Independence support: you pay a smaller contribution
  • Everyday living: you pay a higher contribution
    How much you pay depends on your income, assets, and pension status. If you do not share your financial details, you may be charged the highest rate.

Transport

  • Help getting to and from activities and appointments

Assistive technology and home modifications

  • Assistive technology to support your independence including Hearing aids, Voice assistance devices, Prosthetics, Safety necklaces and Watches for slip and trip monitoring
  • Home modifications to support your independence including: Grab rails, non-slip flooring, Wheelchair ramps.
Assistive Technology and Home Modifications (AT-HS) are assessed and funded separately by the Commonwealth, based on your individual circumstances. Contact us to discuss your requirements and funding options.

Meals

  • Help with meal preparation in your kitchen
  • Support with grocery shopping
